Good Morning,
As discussed at our Town Hall meetings, Maefair will soon have our COVID vaccine clinics provided by CVS pharmacy.
Our clinics are as scheduled for the two-part vaccination series on January 7th and the 28th from 10am to 4pm.
Attached please find the COVID-19 responsible Party consent form,
It is imperative that we get the consent forms back before January 3rd of acceptance of the vaccination.
We have also included a Q&A about the COVID-19 Vaccine.
Also attached, please find a letter and form from Clara Merva, Director of Social Services, to provide Maefair with updated contact information. It is important for staff to have the most current information and best way to contact families if the need should arise. Please complete the form and return it as well.
